Inquest jury finds that failures by prison staff contributed to the death of Stewart John Stanley at HMP Exeter

Isabel Bertschinger represented Stewart’s former partner and child at the inquest which took place between 17-27 July 2023. Stewart died on 13 July 2020 after ligaturing whilst he was imprisoned at HMP Exeter.

The jury concluded that the risk that Stewart might have taken his own life should have been better appreciated, and that policies designed to reduce such a risk did not work appropriately. Both failures were found to be probably causative of Stewart’s death.
