Inquest finds contributory failing in treatment of 14 year old girl by the Tavistock Trust.

An inquest into the death of a 14 year old girl who was receiving out-patient treatment from the Tavistock Trust concluded on Friday 3 February 2023 at Barnet Coroner’s Court before HMAC Murphy. The Coroner ruled, after hearing the evidence in the case, that the enhanced investigative duty was engaged arising from arguable breaches of the Article 2 systemic duty.

The inquest concluded that the young girl’s death was contributed to by an omission of her community mental health team to inform her mother that she had stopped taking her prescribed mood-stabilising medication.

Angelina Nicolaou represented the family at the inquest, instructed under a licence through Advocate.
