Inquest concludes multiple failures contributed to the death of June ‘Angie’ Challis

Angelina Nicolaou, instructed by Leanne Devine at Scott Moncrieff Solicitors, represented Angie’s family at the inquest into her death. She died on 22 August 2022 after swallowing an item on the Chartwell Ward, in Priority House. At the time, Angie was detained under the Mental Health Act, under the care of Kent & Medway NHS Trust.

An inquest jury has concluded that failures in risk assessment, documentation, support, and observations contributed to Angie’s death. Notably, the jury concluded that the failures in respect of carrying out observations were due to staff shortages.
